This series of written and video Case Studies is focused on primary care experiences implementing the 4Ms (Mentation, Medications, Mobility, What Matters) of Age-Friendly Health Systems (AFHS) in their practices. Case study authors participated in the 2020 and/ or the 2023 Geriatrics Workforce Enhancement Coordinating Center (GWEP-CC) AFHS Action Community. The Institute for Health Care Improvement (IHI) has recognized these practices as an Age-Friendly Health System Participant (Level-1) or Age-Friendly Health System – Committed to Care Excellence (Level-2).

This product is available to all GCO users. The Table of Contents for the Case Studies includes the participating GWEP and partner, as well as the focused topic related to the 4Ms within their AFHS journey. Select a case study to download it. The Case Studies Video section links to three videos developed in partnership between the GWEP-CC, IHI, the Penn State GWEP, and the Primary Health Network (PHN).
