DESCRIPTION

The Multimorbidity Toolkit is a free compendium of geriatrics education tools and resources that address unique issues involved in managing multimorbidity issues in older adults. The Toolkit includes a portfolio of educational tools that support physicians who want to improve their care of this patient population. This Toolkit was supported by funding from AHRQ.

Also available is an App (for both Apple & Android devices) that is based on our current efforts to address the challenge of providing optimal care for frail elders with multiple chronic conditions, or multimorbidity.
