DESCRIPTION

This webinar is based on the Multiple Chronic Condition Action Steps developed by the American Geriatrics Society. This presentation is based on actual encounters between patients and clinicians and is designed to model and teach the knowledge and skills needed to participate in this approach to decision-making and care. The webinar is complementary to a curriculum developed by the American College of Physicians (available here) which provides web-based teaching, scripts, and patient scenarios to prepare patients and clinicians to communicate priorities-aligned care between patients and clinicians, and across clinicians.
