Led by experienced AGS CoCare®: HELP sites and leaders, this 3.5 hour virtual workshop is an essential opportunity for anyone considering implementing the AGS CoCare®: HELP model at their hospital to learn more and obtain next steps for implementation. 

In this workshop, participants will:

• Learn about the AGS CoCare®: HELP program model and resources available to help with implementation
• Discuss how to make the business case for implementing AGS CoCare®: HELP
• Explore the intersection of AGS CoCare®: HELP & Age-Friendly Care and how the AGS CoCare®: HELP protocols meet the 4M's
• Gain insights from sites who have had successful implementation and overcame challenges
• Participate in open discussions and Q&A

This activity is eligible for up to 3.5 nursing contact hours.
