DESCRIPTION

The GWEP-CC Pre-Conference Session is an annual meeting that brings together the GWEP grantees, HRSA, The John A. Hartford Foundation, and AGS staff . Topics presented include: policy recommendations from Trust for America’s Health to better ensure access to the COVID-19 vaccine for the homebound, articulating What Matters to patients from the 4Ms Age-Friendly Framework, increasing dementia screenings from the CDC’s BOLD Public Health Center of Excellence on Early Dementia, and how GWEPs can collaborate with Project ECHO. There were also opportunities, through virtual break out rooms, for the GWEPs to network and share with each other related to the topics presented. Following the GWEP-CC Preconference Session, members of the National Association for Geriatric Education (NAGE) will provide updates.

Learning Objectives: (1) increase participants’ understanding of strategies for working with state and local agencies and organizations to reach homebound older people with the COVID-19 vaccine; (2) provide opportunities for GWEPs and partners to share learnings from implementation of different strategies to advance age-friendly care of older people in their GWEP; and (3) provide practical support to accelerate participants’ focus on improving care of older people in their communities through education.
