DESCRIPTION
This series of Case Studies is focused on primary care experiences implementing the 4Ms (Mentation, Medications, Mobility, What Matters) of Age-Friendly Health Systems (AFHS) in their practices. Case study authors participated in the Geriatrics Workforce Enhancement Coordinating Center (GWEP-CC) AFHS Action Community (2020-2021). The Institute for Health Care Improvement (IHI) has recognized these practices as an Age-Friendly Health System Participant (Level-1) or Age-Friendly Health System – Committed to Care Excellence (Level-2).
About the GWEP-CC: The GWEP-CC, led by the American Geriatrics Society, is supported by The John A. Hartford Foundation, and serves as a strategic resource for the Health Resources and Services Administration (HRSA)’s GWEP grantees. For more information, please contact the GWEP-CC at GWEPCC@americangeriatrics.org.
