DESCRIPTION
The AGS CoCare®: HELP program, formerly known as The Hospital Elder Life Program, is a well-studied, effective, and innovative model of hospital care designed to prevent both delirium and functional decline.
The AGS CoCare®: HELP online educational curriculum consists of 14 self-directed engaging modules developed to assist hospitals and their teams in all aspects of implementing and running a successful AGS CoCare®: HELP program. Completion of the entirety of the curriculum modules offers 15.5 Continuing Nursing Education credits.
AGS CoCare®: HELP provides institutions with a variety of resources to help support the implementation of its program, by providing access to resources, products, training and technical assistance through webinars, toolkits, an online community, and personal coaching calls.
