New Free Webinar on Multiple Chronic Conditions

Another free resource has been added to the MCC GEMS Toolkit, developed by the AGS under a grant from the Agency for Healthcare Research and Quality (AHRQ).  Presented by Cynthia Boyd, MD, MPH and Matthew McNabney, MD, the new recorded webinar focuses on the care of older adults with multiple chronic conditions,  including an overview of the new MCC GEMS App and providing guidance for the application of these tools in practice.  The MCC GEMS App, the accompanying online educational toolkit, and the recorded webinar are all tools to help guide clinicians through an approach to managing patients with multimorbidity.   

Both the recorded webinar and the MCC GEMS Toolkit also have CME available to those who participate.    Add these tools to your library today!
